What tax form do I use to file my taxes if I had a pay out from a pension plan?

    You can’t use 1040EZ for that, you’ll need 1040A (line 12ab) or 1040 (line 16 ab). You should have gotten a form 1099-R for the amount paid out. If you didn’t roll it over into an IRA you probably owe a 10% penalty for early withdrawal unless you’re age 59-1/2 or over
