How to request 401k rollover from a very old employer?

Contact the 401(k) custodian for the old account. You should have statements from them.

Step 1- Talk to new 401k administrator to see if the 401k roll-in is allowed. Ask them how to make the check payable and get a roll-over form from them. Ask if anything else is required such as a qualification letter or signature.
Step 2- Call the old 401k administrator and request the rollover to your new plan. They may require a signature from you or an acceptance letter from the new plan. Tell them to send YOU the check made payable to the other intsitution.
Step 3- Put the check and roll-over form in an envelope and send it to the new 401k administrator (unless other instructions are given.)
